Understanding the Process of Care

At Choice Care 4U, we are dedicated to providing exceptional care and support to those in need. No matter the type of care being provided, we always follow the same reliable approach.

  1. The process begins with a home visit from an expert assessor, who will discuss your needs and determine how we can best assist you.
  2. You will be in control of designing your own personalized care plan, focusing on the things that matter most to you.
  3. We will match you with regular care workers who have completed recognized health and social care training and have been thoroughly vetted. It is important that you feel comfortable and trust your care worker.
  4. Once a start date has been agreed upon, based on your needs, your care worker will provide encouragement and support to help you maintain your independence and find creative solutions to any challenges.
  5. We use advanced care management technology to ensure the highest quality of care, and you and your chosen representatives can access your care schedule, notes, and communication with us through our online Family Portal.
  6. We will conduct regular reviews of your care to ensure it continues to meet your needs over time.
